The following are forms that we hope will expedite your business with the Columbia Police Department in a more efficient manner.  The Columbia Police Department does not take police reports via the internet.  This is to ensure that we can properly determine the most appropriate manner in which to handle your particular situation as well to determine who has jurisdiction of the incident.

The following instructions are provided for the completion of the parade permit application:

1.  Download and complete the application.

2.  Forward the completed application to:

City of Columbia Police Department

3.  Enclose $5.00 to cover the cost of the parade permit.  If submitting a check please make the check payable to the "City of Columbia".

4.  For inquires regarding the application process, call 803-545-3535, Monday through Friday, 8 a.m. - 4:30 p.m.
